Diabetes Initiative
Working together with the clinical services of Codman Square Health Center and Dorchester House Multi-Service Center, the Diabetes Outreach and Case Management Team, provide services that are focused on keeping both the community and our patients, educated concerning the importance of receiving comprehensive care for this life threatening disease.The Diabetes Initiative is well underway as we have secured funding from both the Smith Family Foundation and the Blue Cross Blue Shield Foundation of MA. We are also participating in the Massachusetts Diabetes Collaborative which will assist us in improving the care we provide to our diabetic patients. We seek to provide diabetic patients at both Codman Square Health Center and Dorchester House Multi-Service Center with not only improved clinical care but also with non-clinical wellness programming including exercise, nutrition, stress management, and smoking cessation programs. A Wellness Needs Assessment is currently underway to help us determine the optimal non-clinical services to provide to our patients.
